Community Integrated Living Arrangement (CILA) Closure Choice Form

Policy

This Information Bulletin introduces the Community Integrated Living Arrangement (CILA) Closure Choice Form (IL 462-4450). In the event that a CILA provider has an emergency or unexpected closure, the Independent Service Coordination (ISC) agencies will use this form to capture an individual and/or legal guardian's choice of alternative services.

Process and Procedure

Please refer to Information Bulletin DD.15.052 Revision to Emergency Closure of a CILA Agency, which outlines the entire process to be used when there is an emergency CILA closure. As a part of an emergency CILA closure, the ISC agencies will use this form to offer eligible service options to the individual and/or legal guardian, document their choice of service, and to document their choice of provider. Although some ISC agencies have developed their own form and used a similar process over the years, this form will standardize the process to ensure each individual/guardian is receiving the same options.

Effective Date

This information bulletin and the Community Integrated Living Arrangement (CILA) Closure Choice Form (IL 462-4450)are effective immediately upon release.
