Reporting Suspected Fraud, Waste and Abuse


The Office of Inspector General (OIG), in pursuit of its mission to prevent, detect and eliminate fraud, waste, abuse, mismanagement and misconduct within the Illinois Medicaid System, is making it easier for individuals and employees to report suspect employees, providers and recipients. Our on-line complaint form is now located at www.illinois.gov/hfs/oig.  This portal now allows a Complainant to attach any document(s) that can help substantiate the suspected fraud, waste, abuse, mismanagement or misconduct. In addition, the OIG Fraud Hotline number has been changed to:

The web site and phone number are available for the general public and any State Employee to use regarding suspected fraud, waste or abuse in the Medicaid program, SNAP (Food Stamp), TANF (including child care), and other Illinois public assistance programs. Please remember that per your Employee Handbook and Agency Procedures you are required to report suspected fraud, waste, abuse, mismanagement and misconduct to the OIG.
