Prospective New Provider Agency Orientation Training - Canceled Fall 2017

Note: The October 2017 training is canceled in order to allow time to revise the training program and better assess service needs. Be watching for any updates in 2018.

Important Details

Please see the information below about the training.

  • All attendees must pre-register before attending. See Registration link below.
  • No credit is given for late arrivals or early departures.
  • Participants are required to sign-in and sign-out to receive credit for attending.
  • Please arrive at least 15 minutes early to sign-in and receive a training handout folder.
  • At least two days in advance of the training registrants will receive an email from the Division of Developmental Disabilities to confirm your enrollment, and if applicable, inform you if your name is on a waiting list.
  • If you do not receive an email enrollment confirmation at least two days in advance of the training, please email DHS.BQM@illinois.gov to inquire.

Note:  Prospective Community Integrated Living Arrangement (CILA) Agency Providers seeking basic information about services, you are not required to attend an orientation if you are licensed under another category or meet the exemption requirements.

  • Be watching in 2018 for any updates.  Registration begins at 8 a.m. and ends at 9 a.m. 
  • The Orientation training begins promptly at 9 a.m. and ends at 2:30 p.m.  Please see the Agenda below.

Parking: free in fenced area lot behind building and free street parking on corner of LaPorte and 109th only.

Registration Form and Handout Materials

  • All attendees must pre-register before attending.
  • Each person must complete a Prospective CILA/DT Provider Orientation Registration Form (not available at this time). 
  • Space is limited.
  • If you do not pre-register, entry and a seat is not guaranteed.
  • No credit is given for late arrivals or early departures.
  • Registrations are on a first-come, first-served basis.
  • The registration deadline is close of business in advance of the training (future date to be determined).
  • After completing and submitting your registration form, an automatic confirmation of your registration will appear on your computer screen.  If you register by a cell phone or other device, you may not see the automatic screen confirmation.
  • Registration includes handout materials.
  • Lunch and snacks are not provided.

Attendance Verification

  • Registration is 8:00 a.m. to 9:00 a.m. Training begins promptly at 9:00 a.m.
  • You will be required to sign-in and sign-out to verify attendance.
  • Within a few days after the training and full attendance is verified, the Bureau of Accreditation, Licensure and Certification (BALC) directs the application process, and upon your request, will send you an application.
  • If you arrive late or leave early, a request for an application will not be accepted by BALC.
