Appendix 3 - Homeless Youth Performance Measures & Standards

Performance Measures

  • Percent of youth with a completed Safety Assessment.
  • Percent of youth with an Emergency Care Plan implemented.
  • Percent of youth with a completed Ansell Casey Assessment.
  • Percent of youth with a Case Management Plan developed.
  • Percent of youth with a Case Management Plan Implemented.
  • Percent of Case Management Plans that include strategies to encourage education employment and/or education (High School, GED, Vocational etc.)
  • Percent of Case Management Plans that include the strategies to increase living skills.
  • Percent of Emergency Shelter and Transitional Living Youth who exited the program to stable housing
  • Percent of Transitional Living Youth who exited the program employed and/or enrolled in educational program
  • Percent of pregnant youth that received prenatal care
  • Percent of eligible youth acquiring with one or more new mainstream benefits (SSI, Medicaid, TANF, SNAP, WIC)

Performance Standards

  • 100 Percent of youth will have a completed Safety Assessment. (80% Acceptable)
  • 100 Percent of youth with an Emergency Care Plan implemented. (95% Acceptable. Considers only youth with a completed safety assessment that required immediate follow-up)
  • 100 Percent of youth will have a completed Casey Assessment. (70% Acceptable. Excludes Outreach only youth.)
  • 100 Percent of youth will have a Case Management Plan developed. (90% Acceptable. Considers only youth with a completed Casey Assessment.)
  • 100 Percent of youth will have a Case Management Plan Implemented. (90% Acceptable. Considers only youth with a case plan developed.)
  • 100 Percent of Case Management Plans will include strategies to encourage education employment and/or education (High School, GED, Vocational etc.) (90% Acceptable)
  • 100 Percent of Case Management Plans will include the strategies to increase living skills. (90% Acceptable)
