Dear Community and Hospital Providers:

The DHS/DMH has recently become aware that there were changes to forms related to the admission of individuals to psychiatric hospitals that are posted to the DHS/DMH internet site for provider access. It appears that these changes have caused confusion among Illinois providers due to references specifically to state operated centers. The Division agrees that the changes are unnecessary and confusing, and have begun the process to modify the forms by removing these references.

Because the process of changing the forms will take some time to complete, the Division offers the following instruction in the meantime:

For individuals being treated outside the state operated centers, please strike through references to the state operated center on the form, and then complete the forms. For any provider who may have printed copies of the previous version of the forms which did not contain these references to state operated centers, you may also continue to copy and use those forms until the updated forms are available on the website.

Once the changes have been made and the new forms are posted, we will advise you by means of a Communication Alert that they are available for your use. If you should have any questions in the meantime, please contact your DHS/DMH Regional Office for assistance.
