SFY 2012 PROVIDER PERFORMANCE AND OUTCOMES REPORTS

The Department of Human Services, Division of Alcoholism and Substance Abuse (DHS/DASA) is pleased to offer electronic copies of the SFY 2012 Provider Performance and Outcomes Reports, which are now available through the DHS/DASA Web page. These reports provide data on engagement, retention, and continuity of care performance measures for individual providers, DHS regions, and statewide averages.

The Provider Performance and Outcomes Reports also include information on client outcomes collected from the National Outcome Measures (NOMS). Client outcomes on alcohol and drug abstinence, employment, housing, criminal justice system involvement, and social connectedness are reported at both intake and discharge, and this information is also included in each Provider Performance and Outcomes Report.

Also included in each individual report, are demographics on all clients opened during SFY 2012. To access the SFY 2012 Provider Performance and Outcomes Reports for all DASA-funded providers, complete the following steps:

  1. Go to the DHS Home Page (http://www.dhs.state.il.us/page.aspx?)
  2. Click on the "for Providers" tab;
  3. Scroll down and click on "by Division;"
  4. Click on the "Alcoholism & Substance Abuse Provider Information" heading; and
  5. Under "Related Links," click on the "Provider Reports" heading.
