- Effective immediately, when Notice of Appeal (Form 103) or Request to Withdraw Appeal (Form 65) is received, fax or email the form to BAH within 2 workdays.
To improve the timeliness of pre-appeal hearings, we are revising instructions to include the emailing or faxing of forms to the Bureau of Assistance Hearings (BAH). Effective immediately, when the customer submits a written statement of appeal or Notice of Appeal (Form 103) to the FCRC, email or fax the statement to the BAH within 2 work days. When emailing, send to firstname.lastname@example.org. When faxing, include a fax transmittal sheet and fax to (312) 793-3387.
When a Request to Withdraw Appeal (Form 65) is received, follow the instructions above for forwarding the form to BAH within 2 workdays. If the Form 65 is signed and received less than 2 workdays prior to the scheduled appeal hearing, email or fax Form 65 to BAH immediately on the day of receipt.
Since hearing officers no longer travel to FCRCs, we are removing the reference to giving Form 65 to the hearing officer when they arrive at the office.
