Flow Charts

  • Request for Information and Request for Application Process:  This flow chart is used to describe the process for the provider to become better informed about the needs of the individual making the transition to the community.  This process will help the provider to determine if there is an interest and the capacity to serve an individual.
  • Customer Needs Evaluation Process - This flow chart describes the process used to identify the needs of the individual making the transition from a State-Operated Developmental Center to community living.  This flow chart also lists the types of information needed before beginning to draft the person-centered transition plan.
  • Person-Centered Transition Plan Process - When the needs of the individual have been determined, this flow chart outlines the development of a draft transition plan, which will be shared and approved before moving the individual into their new home and new life.
