Person-Centered Transition Plan Process
Determine community provider
Draft a person-specific transition plan that is made up of the following :
Clinical transition plan
Person-centered description
Additional assessments, as needed
Determine community provider
Needed housing supports
Needed living supports
Needed day services supports
Needed community links
Present draft transition plan to the individual, parents and guardians for approval
Distribute copies of the draft transition plan
Schedule and conduct a transition plan meeting
Process the funding request
Secure the residence, living supports, day services and other needed supports identified in the person-centered description and transition plan.
Complete transition plan and discharge summary
Transfer the individual's personal belongings and medical records
Move the individual to the new home and life
