MR #11.11: Long Term Care Cases, Voter Registration and Other Changes Related to No Wrong Door Implementation
Summary:
Long Term Care (LTC) and Supportive Living Facility (SLF) cases are specialty cases that are served by specific offices. In Cook County, LTC and SLF applications are processed and their case serviced by the Medical Field Operations (MFO) Family Community Resource Center (FCRC). The MFO FCRC also processes hospital applications (Form 2378H) for Cook County.
The MFO FCRC only processes Cook County applications. If the MFO FCRC receives a LTC/SLF or 2378H application from an applicant that resides in another county, they forward the application to the county of residence. The date of the application is the date it was received in the MFO FCRC.
Beginning in April, Region 2 is consolidating LTC and Supportive Living Facility (SLF) cases into 2 FCRCs, Lake and DuPage. The consolidation will be in 2 Phases through 09/2011.
For the remainder of the State, LTC and SLF applications and active cases are served by the FCRC that serves their county of residence.
A new screen is added to the Online Web Application for Cash, Medical, and SNAP. The Your Office Selection screen will appear after the applicant has indicated they want to submit their application. The Your Office Selection screen allows applicants to choose the office they want to process their application with the exception of LTC cases. LTC Web applications will be directed to the specialty office that serves the area where they reside.
When a voter registration application is received in an FCRC for a resident of another county, mail the voter application to the appropriate election office.

. In the middle of the SBE home page is a section titled Search Engines. Click on "Choose Search Option", then "Election Authorities", and finally "Search". Choose the county of residence from the drop down menu to get the contact information for the county election authority. Follow procedures in WAG 22-12-01 under Office Liaison Duties to forward the application.
New Form 85A and Form 85AS is available in Smart Forms and on One Net. The FCRC completes Form 85A or 85 AS whenever a case is transferred to the HFS All Kids Unit.
At the suggestion of FCRC staff, Form 267 and Form 1721 are revised to add Social Security number or proof that you have applied for a Social Security number to both forms. The Form 267 also adds a line to be used for AABD applicants for requesting proof of mortgage payments, contract for deed, tax bills, and deeds.
