Parent Training Survey Procedure Policy Number and Last Update

The parent Training Survey must be completed in its entirety each year at time of enrollment. One form per family should be completed. The Family Services Coordinator will complete this form in an interview format with the family. (02.05.21)
If the family is not interested in any of the areas offered, ask them if they are interested in other areas no mentioned above; if the answer is "yes", document the area of interest in the "other" column.
If a family chooses counseling, substance abuse, or domestic violence, the appropriate coordinator is responsible for ensuring that the family receives assistance.
The Family Services Coordinator (FSC) writes, on each of these three topics, an "R" (for referral) or "WM" (for written material). If the form indicates referral, or both referral and written material, then immediate assistance is required.
At the end of the second week after the center opens, the responses will be tallied (topic that falls into each area). The FSC will give each coordinator a copy of the results. The director and respective service area coordinators are responsible for responding to all the interests that each parent indicated on the form.
The tally is done using a blank form. Going family by family, the interest for each topic is noted. After all the interests for all families have been noted, count the number and write that total in each area.
This tally forms the basis for the Parent Education Planning meeting held with all coordinators and director on the third week (or sooner) after the center opens. Each person at the meeting will bring their own copy of the tally and together decide how the information will be provided to the parents.
As information is provided, e.g. workshops, newsletters, handouts, etc., for each family, go back to each Parent Training survey and note the date and format.
Post an enlarged copy of the Parent Education Plan in an area easily accessible to staff, so they can immediately record information after each activity where parents can see it. Document only group activities (workshops and presentations) not individual information given to parents.
As soon as each coordinator has provided the parents with all the information requested and has it documented, give it to the FSC or designated staff that is responsible for filing the Parent Training survey in the family file. (02.05.27)
