Infants turning 7 months old during the report period.

Report Item: Clinic

  • Data Entry Screen: Program Information (PA15)
    • Data Field: clinic id
      • Selection Criteria:
        Clinic where the mother was active in WIC at delivery.

Report Item: All Infants

  • Data Entry Screen: Program Information (PA15)
    • Data Field: program code open/cert date actual closed/term date program status category
      • Selection Criteria:
        Infants who were active in WIC at their 6 month birthday.
        Women participants who were active in WIC seven months before the report period.
  • Data Entry Screen: Participant Enrollment (PA03)
    • Data Field: date of birth
      • Selection Criteria:
        Infants who were born seven months before the report period
  • Data Entry Screen: Infant/Child Health Visit (PA09)
    • Data Field: type of service
      • Selection Criteria:
        Infants with any WIC service type code.
  • Data Entry Screen: Postpartum Data (PA10)
    • Data Field: actual delivery date pregnancy outcome infant's participant id
      • Selection Criteria:
        Live birth with an actual delivery date seven months before the report period.
  • Data Entry Screen: Birth Data (PA11)
    • Data Field: mother's participant id
      • Selection Criteria:
        Used to link mothers and infants not linked on the postpartum data screen.
  • Data Entry Screen: Participant Group Relationships (PA06)
    • Data Field: participant id group id relationship
      • Selection Criteria:
        Used to link mothers and infants not linked on the postpartum data screen. Relationship must be son or daughter

Report Item: Ever Breastfed

  • Data Entry Screen: Infant/Child Health Visit (PA09)
    • Data Field: type of service ever breastfed
      • Selection Criteria:
        Infants with any WIC service type code.
        Ever breastfed indicator set to yes

Report Item: Total Currently Breastfed

Week 1, Week 2, Week 4 and Week 12

  • Data Entry Screen: Infant/Child Health Visit (PA09)
    • Data Field: type of service ever breastfed date BF ceased
      • Selection Criteria:
        Any WIC service type code. Ever breastfed indicator set to yes.
        Date breastfeeding ceased used to calculate weeks

Report Item: Exclusively Breastfed Infants

Week 1, Week 2, Week 4 and Week 12

  • Data Entry Screen: Week 1, Week 2, Week 4 and Week 12
    • Data Field:  type of service ever breastfed age first formula
      • Selection Criteria:
        Any WIC service type code.
        Ever breastfed indicator set to yes.
        Age first formula used to calculate weeks
