Mail-In Application for Medical Benefits

A completed Form G may be submitted to the Client Assessment Unit (CAU) in place of the 183B for applicants who have a disability, but have not been determined disabled by the Social Security Administration.
Forms A through H are required if they pertain to the applicant.
