What is it?

Nursing Services are in addition to any Medicaid State Plan nursing services for which the individual qualifies.  The need for these services must be documented in the individual's service plan.  Nursing Services must be within the scope of the Illinois Nursing and Advanced Practice Nursing Act (225 ILSC 65).  Nursing Services are only available to individuals in the Adult DD Waiver who are authorized for Home-Based Support Services (HBS).


How Do I Become A Provider?

1.  You must be licensed as a:

  • Registered professional nurse licensed to practice in the State of Illinois or
  • Licensed practical nurse under the direction of a registered nurse who is licensed to practice in the State of Illinois

2.  You must have a National Provider Identifier (NPI).

3.  You must enroll in the IMPACT provider enrollment system as a Medicaid waiver provider. 

Please see the DHS DDD IMPACT information page for more information about enrolling in IMPACT as a DD waiver provider.  Nurses

will enroll in IMPACT as Rendering/Servicing providers. 

After submitting an IMPACT application, please email Provider Enrollment the IMPACT application ID to ensure that the application is reviewed by a DDD Provider Enrollment Specialist.

4.  You must complete a Payee Designation/Authorization Form (IL462-1180).

This form does not need to be submitted to DHS/DDD.  The form should be maintained in the records of the agency that is submitting the

billing for the nursing services. 


How do I enroll my nursing services agency?

Nursing services  agencies must enroll as a Group Practice:

  1. The following documents must be submitted for a Group Practice to Provider Enrollment:

  2. The business is required to have a type 2 - organizational National Provider Identifier Number (NPI).

  3. You must complete an online application for the Group Practice to enroll in the IMPACT provider enrollment system. Please visit the DHS/DDD IMPACT page for additional information specific to the DD Medicaid Waiver Program(s).The Group Practice enrollment type is designated in the DD Medicaid waiver programs for business entities that are providing behavior intervention and treatment services, counseling/psychotherapy, physical therapy, speech therapy and occupational therapies. The business entity is not providing any DD waiver services and is viewed as a billing provider for the individual therapists, counselors, etc. that are providing the Medicaid services. The purpose of enrolling the Group practice in IMPACT is to ensure that all entities billing for a Medicaid provider are properly screened as required by federal requirements. After submitting an IMPACT application, please email Provider Enrollment the IMPACT application ID to ensure that the application is reviewed by a DDD Provider Enrollment specialists.
  4. All Nurses that are working for the Group Practice will need to enroll in IMPACT as a Rendering/Service Provider. On Step 3 of the application, the provider will need to enter the NPI for the Group Practice as the billing provider.
