Appropriate Professional Credentials Not On File

This means that the professional is billing for a service for which they do not have the appropriate credentials.

  • Review Section VII of the Provider Waiver Manual to see what the provider qualifications are for each service. 
  • FAX documentation (copy of license or approval letter from the DDD Bureau of Quality Management) to Provider Enrollment at 1-217-558-2799 if you believe you meet the provider requirements and there is an error on the database. Staff will correct the database.
