Reporting Institutions

What to Report

Admission and discharge information as specified in FOID Reporting User Manual. [from 740 ILCS 110/12(b)(2)] Please complete the following forms and fax to (217) 557-7975 to initiate the registry process.

Who Should Report

Licensed Private Hospital

Any privately owned home, hospital, or institution, or any section thereof which is licensed by the Department of Public Health and which provides treatment for persons with mental illness. [from 405 ILCS 5/1 -113]

Stand alone psychiatric hospitals or those hospitals with behavioral units of certified AMI (acute mental illness) beds.

Long Term Care Facility

Any privately owned home or institution, or any sections thereof, which is licensed by the Department of Public Health under the Nursing Home Care Act and which provides treatment for persons with mental illness. This includes, but is not limited to, those nursing homes designated as "Institutions for Mental Disease (IMDs)".
