Valid prior to January 2, 2014.
FOID Reporting is accomplished via a web based system developed and maintained by the Department of Human Services (DHS). DHS is responsible for comparing the data reported against the State Police FOID files to identify possible matches. The Illinois State Police is then responsible for investigating and processing the application for the FOID card.
The FOID Reporting User Manual (pdf) gives detailed descriptions on the use of the FOID Reporting System for both batch format and manual entry reporting.
Valid after January 2, 2014.
New batch submission file requirements will take effect January 2, 2014 to comply with the new law, P.A. 98-63, pertaining to Concealed Carry in Illinois. Batch Submission Record Layouts (pdf) contains the flat file record layouts.
