Program Description

Target

The vision of this project is to maximize healthy births in two Chicago community areas, Austin and North Lawndale. Three clearly identifiable components of this project are: 1) improved outreach to approximately 600 at-risk, non-engaged-in-service pregnant women in the two communities, 2) increased services for these women, and 3) interconceptional care for women who have had a very low birth weight birth, or a fetal or neonatal loss. In addition, the project targets the postpartum woman who has had a previous very low birth weight delivery or a fetal or neonatal death because research indicates that she is likely to experience a repeated negative outcome at the next pregnancy.

Purpose

The overall objective of this project is to increase healthy births in North Lawndale and Austin by 20%.

Delivery Method

The outreach agency in Austin will enroll up to 263 women annually with no more than 68% of the women being high risk. At least 95 percent of the women enrolled by the outreach agency in Austin must reside in the Austin community.

The outreach agency in North Lawndale will enroll up to 175 women annually with no more than 68% of the women being high risk. At least 95% percent of the women enrolled by the outreach agency in North Lawndale must reside in the North Lawndale community.

Program Effectiveness

By the end of the project, it is anticipated that 75% of unserved at-risk women will have been reached and that the percentage of low birth weight babies for this group will be reduced by 20%. The project is expected to last for four years and serve 2000 women.
