Applying for State Employment
Create a master copy of your application:
* Complete the CMS-100 application leaving boxe #1 and written signature blank.
- Make several copies.
- Fill in boxe #1 and sign the copy for each position you would like to test.
For written tests: thoroughly complete the CMS-100 application; when ready to test, bring the application with you to one of the following test locations:
