The infant must have an 806 service for each of the following intervals active in HRIF: 3 through 5 months, 6 through 7 months, 11 through 13 months, 17 through 19 months, 23 through 25 months.
Face to Face Visits Activity Entry activity date Infant's age in months is calculated for the HRIF start date, the quarter ending date and each face to face visit. The infant must have a face to face visit for each of the following intervals active in HRIF: 3 through 5 months, 6 through 7 months, 11 through 13 months, 17 through 19 months, 23 through 25 months.
Home Visit Activity Entry activity date, site of contact Set to 'Yes' if the infant received a home visit at any time before the quarter ending date.
Developmental Screening Actual Services actual service date, service type code Service type code is 824. Infant's age in months is calculated for the HRIF start date, the quarter ending date and each 824 actual service date. The infant must have an 824 service for each of the following intervals active in HRIF: 2 through 6 months, 11 through 13 months, 17 through 19 months, 23 through 25 months.
Postpartum Depression Screening Participant Group Relationships and Actual Services mother's participant id, infant's participant id, relationship, mother's participant id, actual service date, service type code Set to 'Yes' if the mother, linked to an HRIF infant on the Participant Group Relationship screen, received an 825 service within one year after the infant's date of birth. If the infant is not grouped, the screening is counted as missing. If the infant is grouped with someone other than the mother the screening is not necessary.
