An Application for Circuit Breaker and Illinois Cares Rx (Form IL-1363) is available on the Department on Aging's (DoA) website at http://www.state.il.us/aging/ (click on News & Publications, then select Prescription Drug Assistance, followed by Prescription Drug Assistance in Illinois, then www.illinoiscaresrx.com and Apply Now) or can be requested by calling DoA toll-free at 1-800-624-2459.
If a person needs help in completing the application, they can call the Senior HelpLine at 1-800-252-8966.
HFS's Health Benefits Hotline at 1-800-226-0768 (TTY 1-877-204-1012) can refer seniors to community-based agencies for application assistance.
