An application for Illinois Cares Rx can only be made through the Department on Aging on an Application for Circuit Breaker and Prescription Coverage (Form IL-1363).
A new application must be filed by December 31 of each year. To avoid a delay in receiving benefits, a person should apply well in advance of December 31.
HFS/Illinois Cares Rx handles appeals concerning the denial of prescription medicines. If the Family Community Resource Center receives an appeal involving Illinois Cares Rx, it should be forwarded to the HFS Bureau of Administrative Hearings.
