WAG 26-04-04-a How to Complete HFS 29 and HFS 94

Completes Form 29 or Form 94. The instructions are printed on the back of the form.

Sends Form 29 or Form 94 to the FCRC.

Review the form for:
completeness, including Federal Employer's Identification Number (FEIN) or SSN and zip code in Item 1 and Case Number in Item 7;
meeting time limits;
required documentation;
the deceased's correct charges, assets, and resources; and
a legible signature.

Have the form signed by a person approved to sign vouchers.

Send the original and one copy to the Funeral and Burial Unit, Bureau of Family Community Resource Center Transactions and Support Services. File a copy in the case record.
