1. (FCRC) Look for a court order for proof paternity is established.
  2. (FCRC) Paternity is established for a child born in Illinois after 08/09/96, when a father's name is listed on the official birth certificate from the Department of Public Health, Bureau of Vital Statistics.

    If DPH amends the birth certificate of a nonmarital child born before 08/09/96 to list the name of the father, paternity is established.

  3. (FCRC) Determine if DCSE has sent Adjudication of Paternity (Form 3148) to tell the Family Community Resource Center paternity is established.
