WAG 22-11-03: Spanish Forms and Materials

PM 22-11-03

1. (FCRC) Ask the applicant to complete a Request for Financial Assistance - Medical Assistance - SNAP 2378BS).
   1. Tell the applicant to sign the rights and responsibilities page of Form 2378BS.
2. (FCRC) Insert Form 2378BS in the case record with the other application documents like the screener CAF. 

   NOTE: SLUs are not required to sign the English signature pages.

3. (CO) Generates Spanish notices for all notices sent to the client.

When there is a difference between the AIS and Form 2378BS information, the information on the HFS 2378BS is considered correct.

1. (FCRC) Correct the error, if a mistake was made in transferring information from Form 2378BS to AIS.
2. (FCRC) Correct any underpayments that result from the error.
