Foster Care/Adoption Care

The Department of Children and Family Services (DCFS) handles claims for the death of a child receiving Foster Care/Adoption Care Assistance.

Foster parents may file a reimbursement claim if:

  • they have paid the entire cost of the funeral and burial, and
  • the amount paid is at least as much as the Department's maximum rate.

DCFS sends to the Funeral and Burial Unit, Springfield, within 5 workdays of receipt, either:

  • HFS 29, Funeral or Burial Claim Form, or
  • HFS 94, Funeral and Burial Reimbursement Claim.
