PM 22-05-01

  1. (FCRC) Complete 2 copies of Request for Excess Shelter Allowance (Form 1709).
  2. (FCRC) Send one copy of Form 1709 with a copy of the client's latest Form 552 to the Regional Administrator or designee.
  3. (FCRC) Keep a copy of Form 1709 as control.
  4. (RA) Enters approval or denial on the bottom of Form 1709. If approved, indicates amount of excess shelter allowance.
  5. (RA) Sends Form 1709 to the FCRC.
  6. (FCRC) Complete Form 552 for approved requests. Enter the approved amount shown on Form 1709 in Item 80 using Needs Code 109 ESA (Excess Shelter Allowance).
  7. (FCRC) Complete Notice of Decision on Request for a Cash Assistance Increase/Special Authorization (Form 1934) and send to the client.
  8. (FCRC) File copies of Form 1709 and Form 1934 in the case record.
