- (Provider) Sends Report of Action (Form 2151A) to the FCRC or to the SNAP E&T Office in Cook County when the customer becomes employed, or when other changes in circumstances occur which may reduce or end benefits. If Form 2151A is received for a case that has been transferred to another office, forward the form to the new FCRC.
- (FCRC) Determines continued eligibility based on the information reported on the Form 2151A.
- (SNAP E&T Office) Refers information related to SNAP eligibility to the Mid South FCRC for a determination of continued eligibility.
