1. FCRC (Chicago) Issue client CTA Transit Cards from CTA Transit Card Fund.
  2. FCRC (Chicago) Complete Authorization for Payment of Transportation Expenses (Form 2366) when a CTA Transit Card is issued for transportation.
  3. FCRC (Chicago) Get client's signature on Form 2366 when CTA Transit Cards are given to pay for transportation.
  4. FCRC (Chicago) Approve only enough CTA Transit Cards for up to one month.
  5. (Custodian of CTA Transit Card Fund) Send Transportation Fund Reimbursement Request (Form 1664) when more CTA Transit Cards are needed.
  6. (Custodian of CTA Transit Card Fund) Send Form 1664 and original copies of Form 2366 and Transportation Fund Daily Summary Sheet (Form 2366B) to:

    Emergency Revolving Fund Unit
    100 South Grand Ave. East
    Springfield, IL 62762 

  7. Family Community Resource Center (Outside Chicago) Issue cash for bus fare from petty cash fund.
  8. Family Community Resource Center (Outside Chicago) Approve only enough cash for one month.
  9. Family Community Resource Center (Outside Chicago) Use Petty Cash Receipt (Form 207) when cash is issued.
  10. Family Community Resource Center (Outside Chicago) Get client's signature on Form 207.
  11. Family Community Resource Center (Outside Chicago) Approve additional requests, beyond one month, if needed.
