The following services require prior approval from the Bureau of Comprehensive Health Services:

  • more than 2 surgical procedures;
  • additional surgery within 6 months of original surgery; and
  • items or services not listed on the HFS billing form.

The decision on the request for prior approval must be made within 30 days.
