WAG 19-05-01: REDE of Eligibility

PM 19-05-01

1. The REDE of eligibility for cash/Medical is the contact for SNAP. Use the following forms in the REDE of eligibility for cash/Medical:
   • Redetermination Report Form (Form 643/Form 643CF)
   • AABD Redetermination Report Form (Form 643G)

     The REDE form is proof that the contact was completed. No additional documentation is needed. 

2. Document any changes that have occurred on Form 514  in ACM and save to Content Management. Immediately determine the effect of any change on the benefit amount.
3. If the cash or medical case is canceled because the client did not cooperate with the REDE, cancel SNAP benefits along with the cash or medical case.
