When a resident leaves a group home, their staff can no longer act as the client's authorized representative.

When the client leaves, the group home must:

  • give the client a change report form; and
  • tell the client to report their new address to the Family Community Resource Center within 10 days; and
  • notify the Family Community Resource Center in writing that the client has left.

If the client does not contact the Family Community Resource Center with their new address, send a request for the information to their last known address before they entered the group home. If the client does not respond within 10 days, take action to cancel their case.
