The group home must submit a monthly list of residents receiving SNAP benefits to the FCRC serving the area by the 5th of each month. The group home may use Monthly Activity Report for Drug/Alcohol Treatment Facilities or Group Homes (Form 3037) for this purpose. A group home is not required to use Form 3037.
The list must include:
- The case name and number of each resident who received SNAP during the month.
- A signed statement by the group home administrator that the list is complete and correct.
