Once a resident leaves a treatment facility, the facility's staff cannot act as the client's authorized representative delete text or use their SNAP benefits.

When the client leaves, the facility must:

  • give the client a change report form; and
  • tell the client to report their new address to the Family Community Resource Center within 10 days; and
  • notify the Family Community Resource Center in writing that the client has left.

If the client does not contact the Family Community Resource Center with their new address, send a request for the information to their last known address before they entered the facility. If the client does not respond within 10 days, take action to cancel the case.

The facility must give the Illinois Link card and:

  • If SNAP benefits were issued for the month but have not been used on behalf of the client, the facility must give the entire month's SNAP benefits to the client.
  • If any portion of the monthly SNAP benefits were used on behalf of the client, the facility may or may not be required to give the SNAP benefits to the client, as follows:
    • If the client leaves before the 16th of their fiscal month, they receive one half of their monthly SNAP benefits.
    • If the client leaves on or after the 16th of their fiscal month, they receive none of their monthly SNAP benefits.

The facility must keep a receipt showing the amount of SNAP benefits returned to the client.
