Let the appellant/representative examine material which will be presented at the hearing. Material must include:
Statement of Facts (Form 102);
pertinent case information and all documents to be used at the hearing;
information related to the budget; and
information, medical records, etc. used by CAU to reach a finding of "not disabled", if appropriate.

Use "IM Change Notices Issued Centrally/Intake Notices Issued Centrally" listing at the appeal hearing to verify:
that a notice was sent and
the date of the notice.

Present as testimony at the hearing, the actual date that the action under appeal was taken and the TA and TAR used to generate the central notice. TA and TAR codes identify the message printed on the centrally issued notice.

Present as evidence at the hearing:
a copy of the listing for the client notice under appeal; or
the Form 552 used to process the action; or
the ACID clearance showing the action taken.
(RRP Unit)
Testifies and explains the restriction decision when a decision was to restrict the client to a primary care provider.
(CCR&R/BCCD)
Testifies by telephone and explains the child care decision.
