1. (FCRC) For TANF Cash cases, complete and mail a Notice of Decision on a Request for Financial Assistance Increase (Form 1934), when a decision is made on a written request for increased cash benefits or special authorizations.
  2. (FCRC) For TANF Cash cases, complete and mail a Notice of Decision on Request for Crisis Assistance (Form 2690), when a decision is made on a written request for Crisis Assistance.
  3. (System) For all cash programs, sends a Notice of Decision on Request for Medical Service/Item (Form 2352), to the client when a medical prior approval request is approved or denied.
  4. (System) Sends the Family Community Resource Center a copy of Form 2352:
    • for all denials;
    • for approvals of a lesser or different service/item than that requested; and
    • for approval to borrow prosthetic devices and/or medical equipment that HFS owns.
  5. (FCRC) Upon receipt, file Form 2352 in the case record.
