For TANF, GA, and AABD cases, when information is released for any purpose, document in the case record the following:
- the name of the person requesting the information;
- the agency represented;
- the date of the request;
- the date of the release of the information;
- the specific information released; and
- the reason for the release.
Make an entry in the case record when any request for case information is denied. Explain the reason for denial.
A Family Community Resource Center administrator may, if they desire, require supervisory approval of the decision to approve or deny a request to release case information.
Use Consent to Release Information (Form 34) to obtain an applicant's or client's permission before releasing any information.
