To be eligible to extend your FY18 program grant for FY19, all grantees must complete the Pre-Qualification Process, Internal Control Questionnaire, Programmatic Risk Assessment and Grant application. The Grant Application and Programmatic Risk Assessment are to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all Bureau grantees must submit for each grant program the following documents:

  1. Grant Application - Grant Application (pdf)
  2. Program Narrative (docx)
  3. CSA Budget/Tracking System and Registration Information
  4. Pre-Qualification Completion documentation

The completed documents are to be emailed to the Division email address (DHS.DDDBCR@illinois.gov) with the subject line stating:

  • Last 4 digits of CSFA #
  • Provider Organization Name

For example, 0190 Jane's ISC agency or 0190 HOPE agency - ISC Program 500

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
