08.05.03 - Child Care Collaboration, Notification of Eligibility

Policy Statement:

The Department or its agents will notify applicants, in writing, of their acceptance into the Child Care Collaboration Program, and subsequent eligibility to use the accompanying policies detailed in Section 50.610(e), within 60 days after receipt of the provider’s application. Providers that disagree with the eligibility determination may apply for reconsideration by writing to the Chief of the Department’s Bureau of Child Care and Development at the Illinois Department of Human Services Child Care Bureau, 400 W. Lawrence, Springfield, Illinois, 62762 within 60 days after notification of the original determination. The Chief of the Department’s Bureau of Child Care and Development will make the final decision on eligibility for the program.

Procedure:

The Head Start Collaboration Director will make a determination of eligibility and will notify the applicant of it’s eligibility status in writing within 60 days.

  1. Approval

    If approved, the Child Care Collaboration provider will receive a packet of information that contains:
    1. Approval letter,
    2. Collaboration Program Procedures,
    3. Approved Child Care Collaboration Family List,
    4. Provider responsibilities/obligations to maintain approved status,
    5. Information sheet and agreement form for parents participating in the collaboration, and
    6. Annual Report and instructions.
  2. Denial

    A provider who is denied designation as a Child Care Collaboration and who disagrees with the determination may apply for reconsideration by writing to:

    400 W. Lawrence, 3rd Floor
    Springfield, IL 62762

    Application for reconsideration must be received within 60 days of the notification of the original determination. An application for reconsideration must:
    1. Be from the Director,
    2. Be on agency letterhead, and
    3. Detail the reasons why the agency believes the decision should be reversed.

The Associate Director of the Office of Child Care and Family Services will make the final decision on designation of approved Child Care Collaboration Programs.
