19-444-26-1565-01 GA Tobacco Enforcement Program

To apply for a SFY2019 DHS DASA award for the same services you previously provided in the tobacco enforcement program, all grantees must complete the Pre-Qualification Process (see FYI below), and Internal Controls Questionnaire through the Grantee Portal.


All grantees must submit the following documents:

  1. Grant Application GA-1565:
  2. Fixed Rate Budget printed from the CSA Tracking System.

Submit the completed grant proposal by sending an email to DHS.GrantApp@Illinois.Gov on or before April 18, 2018 at 12:00 p.m. Applications must be emailed.

Your organizations name and the Catalog of State Financial Assistance (CSFA) Number and the program contact must be in the subject line.

Specifically, the subject line of your email must be:

Your Organization's Name, 19-444-26-1565, Chemetha Baker

The Grant Application file must be named "Your Organization's Name 19-444-26-1565 GA Chemetha Baker.pdf"

The Budget file must be named "Your Organization's Name 19-444-26-1565 Budget Chemetha Baker.pdf"

After your application is approved you will receive a Notice Of State Award (NOSA), that must be accepted through the Grantee Portal before an agreement can be issued.


FYI - Please be aware of the following pre-requirements related to contracting with DHS. FY 2018 grantees must make sure that all pre-requirements are up to date and that all information is shared with municipal contacts on a timely basis.

  1. Federal Employer Identification Number (FEIN)
  2. Current DUNS number
  3. Current WWW.SAM.GOV cage code
  4. Register/update/authenticate the GATA system in the Grantee Portal as needed
  5. Pre-qualify by completing the Administrative and Fiscal Risk Assessment (ICQ) on the GATA system
