All potential grantees must make certain the following things are completed before IDHS staff are allowed to accept an application packet.
- Register with the Illinois Grant Accountability and Transparency Act Grantee Portal
- Pre-Qualification
All entities must be qualified to do business with the State of Illinois. To be qualified for a grant award, an entity must:
- Have an current DUNS number;
- Have a current System for Award Management SAM.gov account;
- Not be on the Federal Excluded Parties List;
- Be in Good Standing with the Illinois Secretary of State, as applicable;
- Not be on the Department of Healthcare and Family Services Provider Sanctions list.
- Complete a Internal Controls Questionnaire (ICQ) as the Fiscal and Administrative Risk Assessment, completed once, on an annual basis. All state agencies will utilize the results of the ICQ.
- Complete a Programmatic Risk Assessment (PRA) for each grant award.
- Complete a Uniform Grant Budget for each grant award. The budget template and instructions can be used as a tool to assist in determining expenses; however, the final budget must be completed in CSA Budget/Tracking System database. The pdf budget or paper copy will not be accepted.
- Click on the link under each column heading for the appropriate needed information. If there is no Notice of Funding Opportunity link found, only an application and a programmatic risk assessment are needed.
