  1. (FCRC) Name a person to forward paternity forms to DCSE.
  2. (FCRC) Give the original, if available, or one copy of each paternity form as required to the named person.
  3. (FCRC) File needed copies of all forms. File one copy of each required paternity form in Permanent/Section 3 of the case record.
  4. (FCRC) Use Administrative Paternity Transmittal Sheet (Form 3002), to forward the paternity forms packet to DCSE.

    Downstate: Send packets to the Paternity Establishment Liaison of the child support regional office serving the Family Community Resource Center.

    Cook County: Send packets to Cook County Field Operations. 

  5. (FCRC) Keep a copy of the transmittal form in the Family Community Resource Center for 12 months.
