  1. (Provider) Sends Report of Action (Form 2151A) to the FCRC when the client becomes employed, or when other changes in circumstances occur which may reduce or end benefits.
    1. (FCRC) If Form 2151A is received for a case that has been transferred to another office, forward the form to the new FCRC.
  2. (FCRC) Determine continued eligibility based on the information reported on the Form 2151A.
