- (Provider) Sends Report of Action (Form 2151A) to the FCRC when the client becomes employed, or when other changes in circumstances occur which may reduce or end benefits.
- (FCRC) If Form 2151A is received for a case that has been transferred to another office, forward the form to the new FCRC.
- (FCRC) Determine continued eligibility based on the information reported on the Form 2151A.
