When the FCRC learns of an unreported change of circumstances, review the case for possible overpayment. When information needed to figure the overpayment is not available from the case record:
- (FCRC) Send Form 1721, Instructions to Recipient, to the client to get the needed information.
- (FCRC) Use one of the following forms to get information from another source:
- Form 266/266A, Request for Employment Verification, to an employer,
- Form 541/541CF, Verification of School Attendance,
- Form 2540, Verification of Living With, or
- Form 46, Bank Inquiry, to a bank.
If the information is not obtained within 30 days from discovery:
- (FCRC) Complete Form 2755, Investigation Referral, to ask the Bureau of Investigations (BOI) to help get the information.
- Include on Form 2755:
- the case ID information,
- a summary of the situation and what you did to try to get verifications, and
- the caseworker's name and phone number.
- Include with Form 2755 copies of the information that supports the belief an overpayment was made.
- (FCRC) Distribute Form 2755:
- original to the Financial Recovery Coordinator (FRC),
- copy in the case record.
- (FRC) Sends Form 2755 to BOI, if complete.
- (BOI) Contacts the FRC to ask for any additional information needed.
- (BOI) Completes Form 2518/2518A, Investigation Report, if the requested information cannot be obtained.
- (BOI) Sends Form 2518/2518A, Investigation Report, to FRC when the investigation is finished.
- (FCRC) Complete Form 2518C, Investigation/Disposition Report, describing the action taken.
- (FCRC) Return Form 2518C to BOI, through the FRC, within 30 calendar days after receiving it.
- (FCRC) Complete overpayment referral.
When an overpayment is identified after the case is canceled and the case has not been referred to BOI:
- (FCRC) Use Form 2755 to report the overpayment to BOI.
- (FCRC) Notify the former client using Form 2404 and Form 2404X.
- (BOI) Investigates the overpayment case.
- (BOC) Notifies the former client of the overpayment using Form 2404 or Form 2404C, Change of Overpayment Information, and Form 2404X.
- Distributes Form 2404X:
- original to former client,
- copy to BOC, Field Recovery Unit, and
- copy to Family Community Resource Center for the case record.
- Distributes Form 2404 or Form 2404C:
- copy to BOC, Field Recovery Unit, and
- copy to Family Community Resource Center for the case record.
Welfare Fraud Hotline Referrals
- (
DHS Help Line) Uses Form 1384, Fraud Hotline Referral Record, to refer Welfare Fraud Hotline reports to the Family Community Resource Center.
- Attaches a copy of the hotline report to Form 1384.
- Sends Form 1384 and the hotline report to FRC.
- (FRC) Takes whatever action is needed.
- (FCRC) Complete Section II of Form 1384.
- File completed Form 1384 in case record.
NOTE: A response to the
DHS Help Line is not needed.
When the Family Community Resource Center is not able to verify the hotline allegation:
- (FCRC) Use Form 2755 to refer the case to BOI.
- Include a copy of Form 1384.
NOTE: Referral to BOI meets the Form 1384 completion requirement.
- (BOI) Investigates the case.
- (BOI) Uses Form 2518/2518A to respond to the Family Community Resource Center.
If the case has been transferred:
- (FCRC) Note on Form 1384 the date the case was transferred and the new Family Community Resource Center.
- (FRC) Returns Form 1384 to the
DHS Help Line is not needed.
- Keeps copy of Form 1384.

Cases receiving SNAP with C codes in certain FCRCs are centrally referred to BOC if no earnings were budgeted for the earnings quarter. See WAG 19-06-05, for actions on cases referred to BOC.
