  1. (Provider) Does not have to be enrolled as approved transplant center to be paid for kidney and cornea transplants.
  2. (Provider) Notifies Bureau of Hospital Services, in writing, prior to transplant.
  3. (Hospital Services) Provides letter acknowledging the request.
