WAG 20-22-02: Prior Approval

PM 20-22-02.

Use the same Transportation Prior Approval Request(Form 2239) to approve transportation for a non-employee attendant. When completing the form use the following procedure codes:

• W8484 - Non-employee Attendant, Service Car
• W8485 - Non-employee Attendant, Taxicab
• W8487 - Non-employee Attendant, Medicar

Direct a call regarding transportation for a DCFS ward to the child's DCFS caseworker or to DCFS at 1-800-228-6533.
