  1. (FCRC) Tell client about Healthy Kids Program when they apply.
  2. (FCRC) Give client Healthy Kids: Good Health for Children and Teens (Form 1123).
  3. (FCRC) Help client find Healthy Kids provider, schedule an appointment, or arrange transportation, when they ask.
  4. (CO) Sends Healthy Kids Program (Form 1802) each year to each active case with child under 21.
  5. (CO) Sends client Notice on Child(ren) Due for Rescreening (Form 2286) not less than 10 workdays before next screening should be done.
