The HFS Bureau of Comprehensive Health Services (BCHS) handles prior approval for the following services:

  • contact lenses and related services;
  • a custom-made artificial eye;
  • low-vision devices; and
  • items or services not listed on the HFS billing form.

The decision on the request for prior approval must be made within 30 days.
