A client may change HFS's designated provider. The request must be in writing. A change in HMO status may take up to 60 days to occur.

If one of the following happens, the change can be made for cause:

  • the client leaves the area;
  • the client's condition changes and the physician cannot treat the condition;
  • the physician dies;
  • the provider leaves the medical program; or
  • the provider tells HFS they will no longer be primary care provider.

HFS will decide if the requested change meets the criteria.
