WAG 18-03-01-a: Required Verifications

PM 18-03-01-a.

1. Ask for proof of the following, once a newborn is added:
   • relationship (see WAG 03-05-01);
   • "living with" (see WAG 03-05-03);
   • application for an SSN (see WAG 03-11-00);
   • assets (see WAG 07-01-00) and income (see WAG 08-01-00) for newborn.
2. Accept Record of Birth (Form 2636), as proof of "living with" and relationship. Verification of Living With (Form 2540) is not needed.
   1. Call to verify that the form was submitted by the hospital, if you question the validity of Form 2636.
